To refer someone to The Heart House, please complete the referral form and email it to [referrals@thehearthouse.org] along with any relevant background information that may help our team understand the individual’s current needs and goals.
After submitting a referral, we encourage you to call [phone number] to confirm receipt. Our staff will review the referral to determine whether The Heart House is a good fit at this time and will follow up by phone to discuss next steps.

A few important things to know about The Heart House:

• The Heart House is a non-clinical, peer-led respite and recovery community. Participants must be able to manage activities of daily living independently.

• Length of stay is individualized and based on the person’s goals, progress, and available next-step options.

• Our program is free of charge and designed to support people experiencing homelessness who are seeking positive change related to substance use, mental health, and stability. 

• Recovery and Accountability plans at the Heart House are person-centered and individualized to acknowledge each person’s unique challenges, priorities, and needs
